Application Checklist
- Is your application signed and complete with full names, dates of birth, social security numbers, etc.?
- Did you provide the following:
- Copy of each family member's birth certificate.
- Copy of each family member's social security card.
- Copy of each adult member's driver's license or photo ID card.
- Copy of most recent bank statement if applicable.

Are you over the age of 62? Do you pay out-of-pocket for medical expenses? If yes, provide written verification of payment within the last 12 months. Are you disabled?
